Please make sure to have these complete required documents (issued within the last 3 months) uploaded at the end of the form:
1. Valid ID of Parent or Authorized Representative
2. Letter of Intent
3. Authorization Letter (if applicable)
4. Barangay Certificate of Residency or Certificate of Indigency
5. Medical Certificate or Clinical Abstract
6. Laboratory Request or Treatment Protocol (with quotation)
7. Social Case Study
8. Patient's ID
9. Whole Body Photo of the Patient

Please apply on our form if the patient and immediate family can make it to the interview schedule. The interview for approved applications will take place in Kamias, Quezon City. Thank you!
